David J. Chizek, M.D.

The rebel, free spirit, skilled surgeon, loyal friend and guy with an unmistakable laugh has left us. Born 8/10/1939, South Bend, IN; died peacefully 8/1/2025, Tonka Bay, MN. Preceded in death by parents, Cletus and Mildred Chizek, and niece, Kathy Selby O'Connor. Survived by sister, Nancy Engle, and nieces Pamela Selby Moore, Susan Selby and Betsy Selby Garner. Leaves behind loving friends Peggy Kotila, Beth Scheunemann, Peter Hartwich, Mike Holmquist, Charlie Balgaard, Kris Diller, caregivers Lilian, Esther, Veronica and Neeta, and many others. 

Dave graduated from John Adams High School in South Bend, Indiana University and Indiana University School of Medicine. Completed an internship in Denver, CO, and a residency in ophthalmology at the University of Iowa. Served two years as a major in the U.S. Army as head of the Ophthalmology Department in Okinawa, Japan. Moved to Minnesota, specialized in cataract surgery, and established the Burnsville Eye Clinic, his private practice, in Burnsville, MN. Retired in 2001 after a wonderful 35-year career.

In Dr. Chizek's words, "The greatest accomplishment of my life was receiving a diploma from Indiana University School of Medicine. It opened up opportunities for me to enhance thousands of people's lives by significantly improving their vision and quality of life. Being in a position to help others and make them happier, in my opinion, is the ultimate goal and gratification in life." Reflecting his gratitude and desire to help future physicians, he made generous donations to scholarships for students attending Indiana University School of Medicine.

A staunch environmentalist, Dave generously donated to non-profit organizations that reflected his concern for the Earth and all its inhabitants. He lived an active life and enjoyed windsurfing, cross country skiing, bicycling, running, boating and dancing.
